As it's been said, Nightmare takes on forms when he is present in a Nightmare that has been induced by the Toxic Smoke that flows from his body. He is not always present but when he is there are many different forms the nightmare makes him take. Sometimes he can be himself only more destructive looking, other times he can be a creature or critter from the forest, sometimes he can be a truly beautiful beast, many times he comes in the form of a friend, loved one, or past tormentor just more corrupted.

No matter what his form is, they always share traits from his true self. The Nightmare in a dream will take on the form but often times keeps the skull like face, feathers and matted fur appearance. Everything else will either look like the friend, loved one, etc. or whatever it is he is taken the shape of. In the end, he's all the same. Just in a non-physical form.
