I don't know if there's any one thing that makes something realistic, maybe the combination of lighting, posture, colour palette, etc. If you are having difficulty you may want to look at photos, paintings or things from life. I guess the same goes for drawing heads too (although this one is only a profile, hehe) What I found very helpful in drawing the head was first copying pictures of skulls and facial muscle structure. That way you get a better idea of where all the bumps and dents go on a face and why they're there.
When I draw from my imagination I'm usually not that great at realism, because I can't remember all the little nuances of anatomy. It takes much more experience to draw from the mind than what I have currently, so for now you may want to copy medical illustrations, old master artwork (I am fond of Raphael ) and photographs. Or find a mirror and draw yourself, even!
That makes Two blind deer in the forest! lol! I love this 21! Its just amazingly wonderful! Its so dark, and eery. Yet beautiful and graceful. The trasitions from dark to light are astounding... And add a chilling feel... WOW!
I love the facial expression, very wary and almost mournful.
